We honor VISA & MasterCard and also accept most insurance plans and for your convenience, will bill your insurance company for you. We participate with both Idaho and Washington Medicare. Payment is due on date of service.

For elective services not covered by insurance, quick and confidential financing is available through our Vision Fee Plan. If you have a billing question about a future or existing account with us, you are invited to call our specialists in the billing department at (208) 882-4662.

To expedite your check-in, please download and complete the Patient Information  and the Health History forms in advance of  your scheduled appointment.

If you are interested in a complimentary laser vision screening, feel free to bring with you a completed LASIK Vision Health Questionnaire on the day of your visit.

If you would like to transfer your medical records from your previous eye doctor's office, a Medical Records Request is also available for printing to simplify this process.
